TRACKS
A Transition to Adulthood
Program
Attaining
self-sufficiency and assuming responsibility
for their lives is a gradual process for
young people. Many parents need help structuring
this transition for their child. "TRACKS"
is a year round program for youth and young
adults that require assistance to help facilitate
the transition to adult life. The program
follows the Transition to Independence Process
(TIP) System, which is an evidence-based
supported practice that demonstrated improvement
in real-life outcomes for youth and young
adults with emotional/behavioral difficulties.

Eligibility
Requirements:
Youth and
young adults (ages 14-24) living with severe
emotional and behavioral disabilities and/or
a psychiatric diagnosis are eligible to
participate in TRACKS.
